What you need to know…

• Before your first appointment, please familiarize yourself with your insurance plan and bring a copy of your  insurance card and photo identification with you.

• We accept most major insurance plans; however, following the verification of insurance benefits with your carrier; we will notify you regarding any deductible or co-insurance payments.

• Payment at the time of service is expected for co-pays, deductibles, and outstanding balances.

• A minimum fee for completing disability and FMLA paperwork will be charged, but appropriate work excuses will be issued at no charge.
