First Visit

Thank you for choosing Wiregrass Surgical for your medical needs.
Below you will find information and documents necessary for your first appointment with us.

New patients, please make sure to arrive 30 minutes prior to your scheduled appointment and be prepared to provide the following information:

  1. New Patient Forms
  2. Driver’s License
  3. Insurance Card(s)
  4. List of medications currently prescribed or being taken
  5. Name and phone number of your pharmacy
  6. Referral if insurance requires (TriCare, Medicaid, UHC, all exchange plans).
  7. Name of referring physician and/or primary care doctor
  8. A copy of all relevant medical records (images, labs, referring physicians notes, etc.).

These documents will become part of your permanent record and will be stored safely and securely at our office.

A Center of Excellence is a designation bestowed upon medical practices that meet or exceed quality measures and standards for care, usually awarded by a governing body—such as a medical society or association. These Centers of Excellence communicate to the public that the physicians, staff or facility provides leadership, best practices, research, support and/or training for a focus area. Wiregrass Surgical is recognized as a Bariatric Surgery Center of Excellence by the American Society of Metabolic and Bariatric Surgery (ASMBS) and is nationally accredited IAC Vascular Lab facility.

When a practice or facility has been designated a Case Observation Site and Teaching Facility, it usually means that it has been recognized for displaying a high volume of success with specific surgical procedures, resulting in it being a training facility for other surgeons to learn advanced techniques. Wiregrass Surgical is the only accredited Case Observation Site and Teaching Facility in the Southeast for hernia repair by Intuitive Surgical, the makers of the most advanced surgical robot in the world, the da Vinci robot.

Each procedure is different. Prior to your procedure appointment, a doctor or technician will talk with you about what to expect, and the preparation necessary for your procedure or screening.

After your procedure, a surgeon will brief you and/or your family on the progress of your operation and review areas of concern, if any. The results of your procedure will be shared with you via your doctor in a follow-up appointment if necessary or sometimes simply by phone. More often than not, we are now able to help patients avoid a return visit to our office due to the increasingly minimally invasive modalities we use.

Possibly, however we make every effort to ensure you are comfortable during your screening or procedure. While you may experience some discomfort, routinely we employ an anesthetic to reduce any soreness or pain.

All medical interventions and treatments come with some degree of risk. Our job as medical practitioners is to weigh the potential outcome and the risk together to make recommendations for your care. All risks and benefits will be discussed with you so you can ultimately make the best decision on how to move forward.

Some patients are able to provide their own transportation to and from our facility the day of their operation. However, depending on your procedure, we may ask that you arrange for a person to provide transportation before and after surgery. If your procedure requires anesthesia, you will be required to provide alternative means of transportation.

Calling the number listed on your bill will direct you to a representative who can assist you. You may call our office during regular business hours and speak directly to an account representative that will be happy to help answer any question. Our Director of Benefits and/or Financial Director are routinely available.

Additional forms you may need to expedite and streamline the check-in process. We ask that you please print and complete the following forms prior to your appointment:

Insurance

Wiregrass Surgical accepts most major forms of insurance plans and works directly with your insurance administrator.
Check with your insurance company to ensure if your elective procedure is covered under your plan. If you do not have a case manager assigned to you, a Wiregrass Surgical representative familiar with your account will help you with financial counseling and processing claims with your insurance provider.

Please remember that all co-pays, deductibles and past due balances are due at the time of service. All co-pays and deductibles are due in advance of a scheduled procedure. Our Financial Director will work hand-in-hand with you to pre-determine your benefits and any out-of-pocket expenses you may be responsible for.
